Thank you for considering Diversity Wellness Center. We accept referrals for adults who may benefit from Adult Rehabilitative Mental Health Services (ARMHS).
We are here to help individuals build skills, confidence, and stability to live more independently and successfully in their communities.
Who Can Refer?
We welcome referrals from individuals, family members, case managers, therapists, social workers, healthcare providers, county workers, and community partners.
Our team will review the referral information and follow up regarding next steps, eligibility, and service availability.
